2019 ALBERTA MASTERS GAMES - PARTICIPANT’S WAIVER OF LIABILITY AND RELEASE OF ALL CLAIMS
PLEASE READ THIS DOCUMENT CAREFULLY. THIS IS A LEGAL AGREEMENT BY AGREEING TO THIS WAIVER AND RELEASE YOU ARE GIVING UP YOUR RIGHT TO SUE
LIABILITY WAIVER AND RELEASE: In consideration of acceptance of my participation in the 2019 Alberta Masters Games, I waive any and all claims I may have against, and release from all liability, and agree not to sue the Town of Rocky Mountain House, Clearwater County, the 2019 Alberta Masters Games Host Society, Alberta Sport Connection and any associated Provincial Sport Organizations and their respective directors, officers, officials, servants, agents, sponsors, employees, and volunteers for any personal injury, death, property damage or loss sustained by me as a result of my attendance at and participation in the 2019 Alberta Masters Games arising out of any cause whatsoever including without limitation negligence or breach of statutory duty.
I agree to indemnify and hold harmless the Town of Rocky Mountain House, Clearwater County, the 2019 Alberta Masters Games Host Society, Alberta Sport Connection and any associated Provincial Sport Organizations from any and all third party claims, demands, actions or costs (including legal costs on a solicitor-client basis) for which I am legally responsible arising out of or in consequence of my attendance and participation in the 2019 Alberta Masters Games.
Should medical attention be required while attending an Alberta Masters Games event or function, I authorize the 2019 Alberta Masters Games Society, Alberta Sport Connection or associated Provincial Sport Organization to provide or obtain necessary medical care, including transportation to a medical facility. I accept that I am responsible for any costs associated with that care and transportation.
In entering into the Waiver and Release, I am not relying on any oral or written representations or statements made by the Town of Rocky Mountain House, Clearwater County, the 2019 Alberta Masters Games Host Society, Alberta Sport Connection and any associated Provincial Sport Organizations, including those in any brochures issued to induce me to take part in the 2019 Alberta Masters Games.
RISK: I, the undersigned understand and acknowledge that participation in the 2019 Alberta Masters Games might result in personal injury, property damage or loss, and possible death. I fully understand these risks and hereby agree to participate in the 2019 Alberta Masters Games voluntarily and at my own risk. I further state that I am in proper physical condition to participate in these Games.
RULES: I understand that the rules and regulations are designed for the safety and protection of participants and hereby agree to abide by the rules and regulations set down by the 2019 Alberta Masters Games Society, Alberta Sport Connection and the Provincial Sport Organizations.
MEDIA: I understand that my full name and/or photos of my image may be used in broadcasts, telecasts, on websites and social media, or in written publications related to the Alberta Games.
***CODE OF CONDUCT***
The Alberta Masters Games (AMG) are directed by the highest standards of good sportsmanship and fair play. Therefore, participation demands high standards of personal conduct on and off the field of play. All participants shall:
a) Comply with competition/sport rules and standards set out by Alberta Sport Connection (ASC), the respective Provincial Sport Organization (PSO) and the Host Community;
b) At all times, show respect towards all those involved with the AMG including teammates, opponents, technical officials, volunteers and spectators. This includes all forms of social media and other forms of communication;
c) Consume alcohol only in approved or licensed areas, never arrive at the venue intoxicated, drink alcohol on the field of play or be intoxicated at any official Games event, venue or social function;
d) Promptly inform the venue medical team of any injury/accident sustained as a result of participating in an AMG activity;
e) Respect Games policy stating that all Games venues be non-smoking.
Any actions deemed inappropriate or to be in breach of the code will be brought to the attention of an official Games representative and be assessed relevant to the rules and conduct of the sport and the spirit of the Code. Alberta Sport Connection will be notified of any breach or alleged breach in order to determine if the situation is to be escalated to the Alberta Games Disciplinary Committee prior to imposing an appropriate sanction towards the participant(s) involved. The Games Disciplinary Committee will consist of the Alberta Games Host Consultant, a representative from the Host Society and the Provincial Sport Coordinator.
By clicking below, I confirm that I am of the full age of 18 years, and have read, understood and agree to the contents of this Waiver and Release in its entirety and agree that this Waiver and Release will be binding upon me, my heirs, next of kin, executors, administrators and successors. I have also read, understand, and agree to abide by the Code of Conduct while traveling to, participating in, and returning from the 2019 Alberta Masters Games. I understand that if I do not abide by this Code of Conduct, disciplinary sanctions may be imposed against me by the Alberta Games Discipline Committee.
